Gynecomastia is a benign enlargement of breasts in a male. It usually involves both breasts but sometimes only one breast may be enlarged. It may be’seen in men of any age and is characterized by
– Excessive localized fat deposits
-Excessive glandular tissue enlargement
-Combination of both
Gynecomastia is caused due to an imbalance in the level of testosterone and estrogen. Both these hormones are normally present in a male. The male breast contains both fat and glandular tissue and these tissues can proliferate under hormonal influence. In most cases gynecomastia is physiological and occurs around puberty. Pubertal gynecomastia generally resolves in about 6 months to two years. Non-physiological causes include use of some medications, hormonal or liver diseases etc. or it can be idiopathic in which no cause can be determined.

Surgical management of gynecomastia can involve liposuction (for the fat deposits), excision of the glandular (firm and dense part) or a combination of both of these procedures. Liposuction involves sucking out the fat deposits via small tube inserted through 3-4 mm incisions. If there is an excess of skin it may also need to be tackled either at the same time or maybe at a later date. The surgery is generally done under general anaesthesia and takes around 90 minutes to 2 hours.
Rarely, there may be placement of tube to drain any fluid from the surgical site which will get removed in a day or two. In the post operative period, you will be advised to wear a pressure garment for 3 – 6 weeks and to limit your activities for about 2 days. You will be advised to refrain from strenuous activities for about 3 weeks.
