Gynecomastia is a benign enlargement of breasts in a male. It usually involves both breasts but sometimes only one breast may be enlarged. It may be’seen in men of any age and is characterized by

-Combination of both

Causes

Gynecomastia is caused due to an imbalance in the level of testosterone and estrogen. Both these hormones are normally present in a male. The male breast contains both fat and glandular tissue and these tissues can proliferate under hormonal influence. In most cases gynecomastia is physiological and occurs around puberty. Pubertal gynecomastia generally resolves in about 6 months to two years. Non-physiological causes include use of some medications, hormonal or liver diseases etc. or it can be idiopathic in which no cause can be determined.

Who Requires Surgery for Gynecomastia?

In case of puberty related gynecomastia, it may resolve in some time but if it persists for more than two years it will require surgical management. In an overweight/obese patient, where the breast enlargement is mainly due to fat deposits, losing weight can lead to reduction in size of the breasts but may still need a surgical procedure for improvement of contour. Treatment of any underlying medical cause may lead to change in size. However, residual breast enlargement will need to be managed surgically.

Preparation Before the Surgery

Your surgeon will take your history and will examine you to know the extent of enlargement and to rule out any underlying disorders, You may be advised some tests for the same and also to assess your fitness for the surgery. If you smoke or consume tobacco in any form it will be advisable for you to stop that at least 3 weeks prior to the surgery.

What Happens During the Surgery?

Surgical management of gynecomastia can involve liposuction (for the fat deposits), excision of the glandular (firm and dense part) or a combination of both of these procedures. Liposuction involves sucking out the fat deposits via small tube inserted through 3-4 mm incisions. If there is an excess of skin it may also need to be tackled either at the same time or maybe at a later date. The surgery is generally done under general anaesthesia and takes around 90 minutes to 2 hours.

What to Expect after the Procedure?

Rarely, there may be placement of tube to drain any fluid from the surgical site which will get removed in a day or two. In the post operative period, you will be advised to wear a pressure garment for 3 – 6 weeks and to limit your activities for about 2 days. You will be advised to refrain from strenuous activities for about 3 weeks.

What are the Risks of This Procedure?

  • Post operatively there will be some swelling and some pain for which you will be prescribed medication
  • There may be some bruising which generally settles down with time
  • There may be numbness in the operated part which is generally temporary but may take up to a year to fully recover
  • Infrequently there may be bleeding and formation of a blood clot under the skin which will require surgery for removal
  • There may be collection of some fluid in the operated area and may need drainage of the same 
  • Infection, which is uncommon, may need intravenous antibiotics and surgical drainage
  • Rarely there may be loss of the nipple and aerola which will necessitate reconstruction for the same
  • There may be some contour irregularities and mismatch of the two sides
  • Scarring at the operative site
